Please make sure to take your time filling out the emergency care form. It is vital that I know your child's health history!
Your child must be current, or on schedule, with their immunizations to remain at school.
If your child needs a prescription medication during the school day, a form must be signed by the doctor and the parent before I can administer the medication. Any over-the-counter medications must be brought to school in the original container or bottle. These meds also may be administered once the correct form is signed by the parent.
